    HYPOTHESIS: Women are deterred from a surgical career owing to a lack of role models rather than lifestyle considerations. DESIGN: Survey. SETTING: University teaching hospital. PARTICIPANTS: Surgery and obstetrics/gynecology attending physicians, residents, and medical students. INTERVENTION: Questionnaire. MAIN OUTCOME MEASURES: Potential deterrents to a surgical career. RESULTS: Men and women had a similar interest in a surgical career before their surgical rotation (64% vs 53%, P = .68). A similar percentage developed a mentor (40.0% vs 45.9%, P = .40). Women were far more likely to perceive sex discrimination (46.7% vs 20.4%, P = .002), most often from male attending physicians (33.3%) or residents (31.1%). Women were less likely to be deterred by diminishing rewards (4.4% vs 21.6%, P = .003) or workload considerations (28.9% vs 49.0%, P = .02). They were also less likely to cite family concerns as a deterrent (47.8% vs 66.7%, P = .02) and equally likely to be deterred by lifestyle during residency (83.3% vs 76.5%, P = .22). However, women were more likely to be deterred by perceptions of the "surgical personality" (40.0% vs 21.6%, P = .03) and the perception of surgery as an "old boys' club" (22.2% vs 3.9%, P = .002). CONCLUSIONS: Men and women are very similar in what they consider important in deciding on a surgical career. Women are not more likely to be deterred by lifestyle, workload issues, or lack of role models. However, the perceived surgical personality and surgical culture is a sex-specific deterrence to a career in surgery for women.

    Zusammenfassung Hintergrund Trotz einer steigenden Zahl an Habilitationen in der Ophthalmologie von Frauen, sind Frauen in akademischen Leitungspositionen und bei Professuren weiterhin deutlich unterrepräsentiert. Ziel dieser Studie war es, den aktuellen geschlechterspezifischen Stand der Karrierewege nach der Habilitation in der Augenheilkunde sowie mögliche Einflussfaktoren zu analysieren. Methodik Eine offizielle Umfrage der Deutschen Ophthalmologischen Gesellschaft (DOG), die an habilitierte Ophthalmolog*innen versendet wurde, erfolgte von August bis September 2024 auf Initiative des DOG-Arbeitskreises Frauen in der Ophthalmologie. Erhoben wurden Daten zu Alter, Geschlecht, Informationen zu Kindern, Kinderbetreuung, derzeitigem Arbeitsplatz und Gründen für den Verbleib und das Verlassen einer Universitätsklinik oder Forschungseinrichtung als Arbeitgeber. Ergebnisse Insgesamt beteiligten sich 168 von 458 habilitierten Kolleg*innen an der Umfrage (♀: 40 %, ♂: 59 %, keine Angabe: 1 %). Bei 79,8 % der habilitierten männlichen Kollegen lebten Kinder im selben Haushalt, bei Frauen nur in 64,7 % (p = 0,030); 81,8 % der Frauen und 32,9 % der Männer nahmen Elternzeit (p < 0,001); 72,3 % der Frauen und 6,3 % der Männer mit Kindern arbeiteten in Teilzeit (p < 0,001). Bei Vorhandensein einer zweiten Betreuungsperson arbeitet/arbeitete diese bei den männlichen Kollegen zu 58 % in Teilzeit und in 15 % gar nicht, während dies bei den weiblichen Kollegen nur zu 16 % in Teilzeit und zu 2 % gar nicht der Fall war (p < 0,001). Für den Verbleib in der akademischen Laufbahn wurden am häufigsten das Gesamtpaket der Vorteile einer akademischen Laufbahn (♂: 57,4 %, ♀: 64,4 %) und am seltensten finanzielle Anreize (♂: 2,9 %, ♀: 2,2 %) als Gründe angegeben. Diskussion Es besteht bei Habilitierten der Augenheilkunde eine erhebliche Diskrepanz zwischen gleich qualifizierten Männern und Frauen in Bezug auf die Übernahme der Kinderbetreuung. Frauen übernehmen offenbar den Großteil der Kinderbetreuung, was ein Grund dafür sein könnte, dass Habilitandinnen seltener akademische Leitungspositionen einnehmen. Abstract Background Despite an increasing number of women completing postdoctoral qualifications in ophthalmology, women remain significantly underrepresented in academic leadership positions and professorships. The aim of this study was to analyze the current gender-specific status of postdoctoral career paths in ophthalmology and potential influencing factors. Methods An official survey of the German Ophthalmological Society (DOG) was conducted from August to September 2024 at the initiative of the DOG working group on women in ophthalmology. Data were collected on age, gender, information about children, childcare, current employment and reasons for remaining or leaving a university hospital or research institution as an employer. Results A total of 168 of 458 colleagues participated in the survey (♀: 40%, ♂: 59%, no information: 1%). Of the male colleagues 79.8% had children living in the same household, compared to only 64.7% of women (p = 0.030), 81.8% of women and 32.9% of men took parental leave (p < 0.001) and 72.3% of women and 6.3% of men with children worked part-time (p < 0.001). If a second caregiver was present, this caregiver worked part-time in 58% of male colleagues and 15% did not work at all. In female colleagues the second caregiver worked part-time in 16% and 2% did not work at all. The overall package of benefits (♂: 57.4%, ♀: 64.4%) was cited most frequently as a reason for remaining in an academic career, while financial incentives (♂: 2.9%, ♀: 2.2%) were cited least frequently as reasons. Discussion There is a significant discrepancy between equally qualified men and women in ophthalmology regarding childcare responsibilities. Women appear to take on the majority of childcare responsibilities, which could be one reason why females are less likely to achieve academic leadership positions.

    Are there gender differences in academic medical career aspirations related to research, mentoring and discrimination? A national cross-sectional study of French medical residents Open PDF

    OBJECTIVES: Academic medical careers remain marked by persistent gender inequalities, despite the growing feminisation of the medical workforce. The objective of this study was to examine whether gender differences exist in key individual and institutional determinants of academic medical career aspirations among medical residents, including research motivation, mentoring, work centrality, self-efficacy, perceived discrimination and stress. DESIGN: This was a national cross-sectional online survey. SETTING: Multicentre study conducted across 36 medical schools in France. PARTICIPANTS: A total of 1570 medical residents (997 women and 573 men) voluntarily participated between November 2022 and February 2023. All participants completed validated self-report questionnaires. There were no exclusion criteria beyond being enrolled in a French residency programme. PRIMARY AND SECONDARY OUTCOME MEASURES: Attitudes towards research (interest, motivation, significance) were measured using the Scale of Attitudes towards Research. Mentoring was assessed with the Mentor-Mentee Perception Questionnaire, work centrality with Hirschfeld and Feild's scale, self-efficacy with the General Self-Efficacy Scale, perceived discrimination with the Sexual Harassment and Discrimination Experiences of Academic Medical Faculty instrument and stress with the Perceived Stress Scale. Gender differences were analysed using t-tests or χ² tests with Holm-Bonferroni corrections for multiple comparisons. RESULTS: Compared with men, women reported lower research motivation (mean (SD) 21.0 (5.5) vs 22.8 (5.5); padj=0.011), lower research interest (27.1 (5.5) vs 28.1 (5.7); padj=0.011), lower work centrality (26.7 (7.0) vs 28.2 (8.1); padj=0.011) and lower self-efficacy (28.3 (5.2) vs 29.9 (5.0); padj=0.011). Women were less likely to report having a mentor (38.5% vs 44.5%; padj=0.02). They also reported substantially higher levels of experienced gender discrimination (22.8% vs 3.8%; padj=0.005), sexual harassment (57.7% vs 18.2%; padj=0.005) and perceived stress (8.48 (3.29) vs 7.27 (3.43); padj=0.011) CONCLUSIONS: Gender differences were observed across several individual and institutional factors associated with academic medical career aspirations. Reduced access to mentoring and greater exposure to discrimination and stress among women may contribute to lower research motivation and self-efficacy. These findings highlight the need for institutional strategies addressing mentoring, workplace culture and equity to support gender parity in academic medicine.

    “Breaking barriers: The power of self-efficacy in combating occupational stigma and advancing gender equity in nursing education” Open PDF

    BACKGROUND: Nursing students may encounter occupational stigma due to prevailing stereotypes and perceptions of nursing as a gendered and undervalued profession. Occupational stigma in nursing can contribute to decreased motivation, limited job satisfaction, and hindered professional growth, potentially impacting students' career choices and commitment to the profession. AIM: This research explores how self-efficacy can mitigate the effects of occupational stigma among nursing students and promote gender equity by encouraging both male and female students to pursue nursing confidently and with a sense of professional pride. METHOD: This study utilized a cross-sectional descriptive research design on 560 nursing students, which was conducted at the Faculty of Nursing, XXX University, XXX. Three validated instruments were used: The Nurse Occupational Stigma Scale, The Gender Equity Scale in Nursing Education, and The Student Self-Efficacy Scale. Multiple linear regression was conducted to identify predictors of occupational stigma and gender equity, with AMOS software used for mediation analysis. FINDINGS: Gender equity was negatively correlated with nurse occupational stigma (r = -0.124, p = 0.003), a positive correlation was found between gender equity and student self-efficacy (r = 0.098, p = 0.020), and nurse occupational stigma had a strong positive correlation with student self-efficacy (r = 0.345, p < 0.001). The structural equation model (SEM) suggests that self-efficacy contributes directly to perceptions of occupational stigma without mediation by gender equity. Significant predictors of gender equity included age (p < 0.001) and gender, with females reporting lower gender equity (p < 0.001), income level (p = 0.029), academic level (third level, p < 0.001), and self-efficacy (p = 0.002). For nurse occupational stigma, predictors included age (p = 0.023), income (p = 0.002), academic level (p < 0.001), and self-efficacy (p < 0.001). RECOMMENDATION: The findings of this study highlight the importance of fostering inclusive and equitable environments in nursing education and practice settings. Promoting gender equity may help reduce occupational stigma, thereby improving job satisfaction and retention rates within the nursing profession.

    Visualization of gender, race, citizenship and academic performance in association with career outcomes of 15-year biomedical doctoral alumni at a public research university Open PDF

    It has long been thought that biomedical doctoral students pursue careers primarily as tenure-track/tenured faculty at research institutions. Recent reports showed, however, that the majority of biomedical doctoral alumni engage in a variety of careers. Wayne State University (WSU) undertook a project to understand the career trajectories of its biomedical doctoral alumni to create programs to better prepare its students for careers in multiple pathways. Data were collected on career outcomes of WSU's biomedical doctoral alumni who graduated in a 15-year period from 1999-2014. Careers were classified into three tiers by Employment Sector, Career Types and Job Functions and career paths were examined by alumni gender, race, U.S. citizenship status, and association with certain academic characteristics. Several statistically significant differences in career paths among all demographics were found. For example, women were more likely to be in teaching and providing healthcare, men in faculty and research; Black alumni pursued careers in Government at higher rates and Whites in For-Profit careers; Asians and non-U.S. citizens spent more time in training positions than others. There was no association of academic characteristics such as GRE, GPA, and Time-to-Degree completion with careers in the two largest sectors of Academia or For-profit. Since our trainees are engaged in this rich variety of careers essential to advancing biomedical science and research nationally, it is imperative for the graduate training community to embrace all careers as successful, and transform the model for biomedical doctoral training to foster student success across this broad career spectrum.

    A phenomenological study of factors influencing the gender gap in physics and other STEM-related fields

    Current reports on the gender gap in STEM suggest factors influencing career choice between the sexes are not inevitable. Recent decades have documented a clear change in the gender makeup of some STEM disciplines. However, in many areas, the gap has persisted with little or no change in the number of women choosing to pursue a career in a STEM-related field such as Physics. According to recent AIP reports1, 2, women comprise 21% of undergraduate, 21% of graduate, and 19% of doctoral degrees in Physics. Many factors have been studied by scholars as contributing to the dearth of women in Physics and STEM3-8. Factors including the lack of role models, discouragement from the media, sex difference in cognitive skill, and unpleasant experiences related to gender-bias in the classroom have all been suggested as potential reasons why women tend not to pursue a career in Physics or other STEM field. Suggestions from some scholars also indicate that women often tend to seek out careers in which they feel they can make a difference. Unfortunately, the size of the gap in many areas has reached a plateau that hasn't appeared to shift significantly over time. This phenomenological study will look at how individual perceptions of STEM (with an emphasis on Physics) as a career option may influence academic and professional pursuits. To that end, formal interviews with practicing professionals and college students from a range of disciplines will be used as a primary data collection tool. This paper will provide a synthesis of the empirical data collected through these formal interviews. Factors that emerge from this synthesis as having a significant influence on one's perception of STEM as a career choice will be further examined and discussed. By focusing on individual perceptions, this study aims to contribute to the existing empirical database of factors that influence career choice and perpetuate the gender gap in STEMrelated fields such as Physics. The themes that emerge may stimulate further research on this topic and provide suggestions for best practices that may contribute to closing the gap. © American Society for Engineering Education, 2014.

    Qualitative Assessment of Academic Radiation Oncology Department Chairs' Insights on Diversity, Equity, and Inclusion: Progress, Challenges, and Future Aspirations. Open PDF

    PURPOSE: A lack of diversity has been observed in radiation oncology (RO), with women and certain racial/ethnic groups underrepresented as trainees, faculty, and practicing physicians. We sought to gain a nuanced understanding of how to best promote diversity, equity, and inclusion (DEI) based on the insights of RO department chairs, with particular attention given to the experiences of the few women and underrepresented minorities (URMs) in these influential positions. METHODS AND MATERIALS: From March to June 2016, we conducted telephone interviews with 24 RO department chairs (of 27 invited). Purposive sampling was used to invite all chairs who were women (n = 13) or URMs (n = 3) and 11 male chairs who were not URMs. Multiple analysts coded the verbatim transcripts. RESULTS: Five themes were identified: (1) commitment to DEI promotes quality health care and innovation; (2) gaps remain despite some progress with promoting diversity in RO; (3) women and URM faculty continue to experience challenges in various career domains; (4) solutions to DEI issues would be facilitated by acknowledging realities of gender and race; and (5) expansion of the career pipeline is needed. CONCLUSIONS: The chairs' insights had policy-relevant implications. Bias training should broach tokenism, blindness, and intersectionality. Efforts to recruit and support diverse talent should be deliberate and proactive. Bridge programs could engage students before their application to medical school.

    The "gender gap" in authorship of academic medical literature--a 35-year perspective Open PDF

    BACKGROUND: Participation of women in the medical profession has increased during the past four decades, but issues of concern persist regarding disparities between the sexes in academic medicine. Advancement is largely driven by peer-reviewed original research, so we sought to determine the representation of female physician-investigators among the authors of selected publications during the past 35 years. METHODS: Original articles from six prominent medical journals--the New England Journal of Medicine (NEJM), the Journal of the American Medical Association (JAMA), the Annals of Internal Medicine (Ann Intern Med), the Annals of Surgery (Ann Surg), Obstetrics & Gynecology (Obstet Gynecol), and the Journal of Pediatrics (J Pediatr)--were categorized according to the sex of both the first and the senior (last listed) author. Sex was also determined for the authors of guest editorials in NEJM and JAMA. Data were collected for the years 1970, 1980, 1990, 2000, and 2004. The analysis was restricted to authors from U.S. institutions holding M.D. degrees. RESULTS: The sex was determined for 98.5 percent of the 7249 U.S. authors of original research with M.D. degrees. The proportion of first authors who were women increased from 5.9 percent in 1970 to 29.3 percent in 2004 (P<0.001), and the proportion of senior authors who were women increased from 3.7 percent to 19.3 percent (P<0.001) during the same period. The proportion of authors who were women increased most sharply in Obstet Gynecol (from 6.7 percent of first authors and 6.8 percent of senior authors in 1970 to 40.7 percent of first authors and 28.0 percent of senior authors in 2004) and J Pediatr (from 15.0 percent of first authors and 4.3 percent of senior authors in 1970 to 38.9 percent of first authors and 38.0 percent of senior authors in 2004) and remained low in Ann Surg (from 2.3 percent of first authors and 0.7 percent of senior authors in 1970 to 16.7 percent of first authors and 6.7 percent of senior authors in 2004). In 2004, 11.4 percent of the authors of guest editorials in NEJM and 18.8 percent of the authors of guest editorials in JAMA were women. CONCLUSIONS: Over the past four decades, the proportion of women among both first and senior physician-authors of original research in the United States has significantly increased. Nevertheless, women still compose a minority of the authors of original research and guest editorials in the journals studied.

    The gender gap in science: How long until women are equally represented? Open PDF

    Women comprise a minority of the Science, Technology, Engineering, Mathematics, and Medicine (STEMM) workforce. Quantifying the gender gap may identify fields that will not reach parity without intervention, reveal underappreciated biases, and inform benchmarks for gender balance among conference speakers, editors, and hiring committees. Using the PubMed and arXiv databases, we estimated the gender of 36 million authors from >100 countries publishing in >6000 journals, covering most STEMM disciplines over the last 15 years, and made a web app allowing easy access to the data (https://lukeholman.github.io/genderGap/). Despite recent progress, the gender gap appears likely to persist for generations, particularly in surgery, computer science, physics, and maths. The gap is especially large in authorship positions associated with seniority, and prestigious journals have fewer women authors. Additionally, we estimate that men are invited by journals to submit papers at approximately double the rate of women. Wealthy countries, notably Japan, Germany, and Switzerland, had fewer women authors than poorer ones. We conclude that the STEMM gender gap will not close without further reforms in education, mentoring, and academic publishing.

    Despite the dramatically increased entry of women into general surgery and surgical subspecialties, traditionally male-dominated fields, there remains a gross under-representation of women in the leadership positions of these departments. Women begin their careers with fewer academic resources and tend to progress through the ranks slower than men. Female surgeons also receive significantly lower salaries than their male counterparts and are more vulnerable to discrimination, both obvious and covert. Although some argue that female surgeons tend to choose their families over careers, studies have actually shown that women are as eager as men to assume leadership positions, are equally qualified for these positions as men, and are as good as men at leadership tasks.Three major constraints contribute to the glass-ceiling phenomenon: traditional gender roles, manifestations of sexism in the medical environment, and lack of effective mentors. Gender roles contribute to unconscious assumptions that have little to do with actual knowledge and abilities of an individuals and they negatively influence decision-making when it comes to promotions. Sexism has many forms, from subtle to explicit forms, and some studies show that far more women report being discriminately against than do men. There is a lack of same-sex mentors and role models for women in academic surgery, thereby isolating female academicians further. This review summarizes the manifestation of the glass-ceiling phenomenon, identifies some causes of these inequalities, and proposes different strategies for continuing the advancement of women in academic surgery and to shatter the glass ceiling.
