    Understanding professional disparities in academic anesthesiology: a single-center gender-based survey study Open PDF

    Background Women in academic anesthesiology face numerous structural and cultural barriers that impede academic advancement and threaten career longevity. Understanding the impact of these challenges can allow steps to be taken to improve gender equity and retention in academic anesthesiology. Methods This study describes a voluntary, anonymous survey distributed to faculty members in the Department of Anesthesia & Perioperative Care at an urban tertiary care academic Health System with a variety of anesthetizing locations across six hospitals. All clinical faculty members in the Department, regardless of gender identity, were invited to participate ( n  = 209). The 57-question survey, which was administered over a 3-week time period in 2024, examined the relationship between demographic variables and professional factors relating to career progression. Quantitative data were summarized with descriptive statistics to compare responses between both gender and career stage peer groups. Using inductive thematic analysis, the authors analyzed open-ended survey data and developed themes that will be used to develop future improvement initiatives. Results The survey response rate was 35% (73/209). Respondents were well-balanced in terms of gender (52.1% women, 45.2% men, 2.7% other) and other demographic groups. Women respondents more often remained at an assistant professor level after 6–10 years of practice (36.4% vs. 0%), reported fewer first or senior author publications if in practice 5 + years (70% vs. 33.3%), and described non-promotable work responsibilities in early- and mid-career time points (85% vs. 67%). Women less often reported receiving an accelerated promotion (33.3% vs. 44.8%); holding departmental (50% vs. 58.6%), institutional (17.9% vs. 35.7%), or national (34.6% vs. 44.8%) leadership positions; and having participated on an editorial board (26.5% vs. 37.9%). More women, particularly in early- and mid-career stages, reported active intent to leave the institution (21.1% vs. 12.1%). Three clear themes emerged from qualitative survey data: 1) Career Advancement, 2) Work Culture, and 3) Work-life Integration. Conclusions Differences are reported in traditional metrics of academic career success between men and women in an academic anesthesiology department. The themes identified provide concrete targets for improvement in improving equity between women and men and retention in academic anesthesiology.

    Exploring gender disparities in academic orthopaedic surgery faculty: analyzing subspecialty and leadership diversity to foster inclusivity Open PDF

    Background The historic gap in gender diversity within orthopaedic surgery is widely acknowledged and continues to persist. The lack of female representation in orthopaedic surgery has been attributed to a variety of factors, including the absence of female mentors and leaders within the field. As such, we sought to examine the gender diversity among orthopaedic surgery faculty in various subspecialties at academic institutions and the distribution of female faculty in positions of leadership. Methods The American Medical Association Fellowship and Residency Electronic Interactive Database (FREIDA) was used to identify all allopathic orthopaedic surgery residency programs during the 2022 to 2023 academic year. The total number of faculty, and distribution of female faculty by subspecialty were collected from January to March 2023. The mean and percentage of female faculty in each subspecialty per program was calculated. Results The total number of orthopaedic surgery female faculty identified was 524. The subspecialty with the highest percentage of female faculty per program was pediatrics at 26.1% (148/511). Hand (18.6%; 113/511), oncology (19.2%; 38/511), foot and ankle (13.6%; 49/511), spine (3.9%; 21/511), shoulder and elbow (7.4%; 7/511) and adult reconstruction (3.7%; 24/511) had lower percentages of female faculty per program. A total of 52 (10.2%) female section chiefs were identified across all programs. Oncology had the highest percentage of female faculty represented in section leadership at 18.4% (7) and sports medicine had the lowest at 4.8% (4). Conclusion Gender diversity of faculty in orthopaedic surgery is low with adult reconstruction (3.7%), spine (3.9%), and shoulder and elbow (7.4%) having the lowest percentages of female faculty. The percentage of female faculty represented in section leadership is also lacking with a total of 52 (10.2%) female section chiefs identified across all programs. Increasing the number of females in leadership positions across all orthopaedic subspecialties may be one step in helping improve gender diversity in the field.

    Engaging and Empowering Future Female Surgeons: Development of the Medical Student Section of the Society of Women in Urology Open PDF

    OBJECTIVE: To describe a replicable model for a large-scale, cross-institutional, and student-driven initiative to promote gender equity within surgical fields. METHODS: To address the lack of a national infrastructure to support female medical students pursuing careers in Urology, a historically male-dominated specialty, the society of women in urology (SWIU) established its medical student section, called SWIUdents, as a student-led initiative to offer mentorship, education, and networking resources for future female urologists. In 2022, SWIUdents convened its first medical student leadership board, comprised of two co-chairs and four subcommittees focused on membership, program implementation, mentorship, and community relations. SWIUdents membership was open to medical students of all gender identities, and recruitment for the organization took place via official SWIU communications, social media, and word of mouth. RESULTS: As of April 2025, there are 319 SWIUdents members, 195 of which are current medical students, representing all regional sections of the American Urological Association (AUA), as well as international medical students. In the 2024 to 2025 academic year, SWIUdents paired 87 medical students with resident or faculty members in Urology. SWIUdents also maintains an active social media presence and offers a broad array of programming, with robust attendance at monthly webinars and journal clubs. CONCLUSIONS: This paper outlines the development and infrastructure of SWIUdents, a national, student-driven model which may be replicated by other specialty organizations aiming to engage and empower women and other groups underrepresented within their respective field.

    Equity in Access to Arthroplasty from a Gender Perspective: A Narrative Review Open PDF

    Objective Despite growing gender diversity in medical education, orthopaedic surgery—particularly arthroplasty—remains one of the most male-dominated subspecialties. This review critically evaluates the barriers limiting female surgeons' access to arthroplasty, including disparities in training, mentorship, professional advancement, and leadership roles. In addition it explores the implications of gender disparities on workforce diversity, patient care outcomes, and surgical innovation while identifying strategies to enhance gender equity in the field. Introduction Women remain significantly underrepresented in arthroplasty, comprising a lower percentage of trainees, faculty, and senior consultants as compared to other surgical specialties. Systemic barriers such as implicit bias, limited mentorship, work-life balance challenges, and inequitable leadership opportunities contribute to this underrepresentation. These disparities influence career trajectories, limiting diversity in surgical perspectives and innovation. Addressing these issues is critical to achieving an equitable and inclusive workforce. Methodology A structured narrative review was conducted using peer-reviewed articles, institutional reports, and orthopaedic society publications. The Data were retrieved from PubMed, Scopus, and institutional databases, focusing on gender representation in orthopaedic surgery, mentorship initiatives, workforce statistics, and leadership opportunities in arthroplasty. Comparative analysis was performed across subspecialties and institutions, and recommendations from diversity and inclusion initiatives were evaluated to highlight successful interventions. Conclusion Achieving gender equity in arthroplasty requires systemic changes, including structured mentorship programs, leadership development workshops, and institutional commitments to diversity. Addressing implicit biases, promoting flexible training pathways, and fostering sponsorship programs for female surgeons are essential. A diverse workforce in arthroplasty will not only promote professional equity but also improve patient care and surgical innovation.

    Authorship Trends and Gender Concordance in Surgical Research

    INTRODUCTION: Building inclusive and diverse research teams is increasingly important in the era of team science. We explored authorship trends over the last 3 decades to assess progress in academic surgery. METHODS: Two general surgical journals (Journal of the American Medical Association Surgery and Annals of Surgery) and two nonsurgical multidisciplinary scientific journals (Nature and Science) were queried at 10-y intervals from 1991 to 2021. Data collected included total number of authors per article, instances of shared authorship, authors' gender, geographic location of the authors, and number of references per article. RESULTS: A total of 741 journal articles were examined. Over the last 3 decades, the number of women authors has nearly doubled in surgical journals and tripled in nonsurgical journals. The proportion of women authors per article, women first authors, and women senior authors all increased significantly, though women remain only 26% of all authors. Intriguingly, articles with a woman senior author had a significantly greater proportion of women coauthors than articles with a man senior author (55 ± 4% versus 12 ± 6%; P < 0.05) across all 3 decades in all journals. In 2021, only 10% of articles with a man senior author had an equal or greater proportion of women coauthors. CONCLUSIONS: Though there are now more women authors, we describe a pattern of gender concordance where women senior authors may publish with other women more than men senior authors. Our findings highlight both the progress made and potential for growth in achieving gender equity in academic surgery.

    Women in Focus: Be Inspired was a unique programme held at the 2019 European Congress of Radiology that was structured to address a range of topics related to gender and healthcare, including leadership, mentoring and the generational progression of women in medicine. In most countries, women constitute substantially fewer than half of radiologists in academia or private practice despite frequently accounting for at least half of medical school enrolees. Furthermore, the proportion of women decreases at higher academic ranks and levels of leadership, a phenomenon which has been referred to as a "leaky pipeline". Gender diversity in the radiologic workplace, including in academic and leadership positions, is important for the present and future success of the field. It is a tool for excellence that helps to optimize patient care and research; moreover, it is essential to overcome the current shortage of radiologists. This article reviews the current state of gender diversity in academic and leadership positions in radiology internationally and explores a wide range of potential reasons for gender disparities, including the lack of role models and mentorship, unconscious bias and generational changes in attitudes about the desirability of leadership positions. Strategies for both individuals and institutions to proactively increase the representation of women in academic and leadership positions are suggested. KEY POINTS: • Gender-diverse teams perform better. Thus, gender diversity throughout the radiologic workplace, including in leadership positions, is important for the current and future success of the field. • Though women now make up roughly half of medical students, they remain underrepresented among radiology trainees, faculty and leaders. • Factors leading to the gender gap in academia and leadership positions in Radiology include a lack of role models and mentors, unconscious biases, other societal barriers and generational changes.

    13-Year Analysis of Gender Disparity in Peer-reviewed Burn Literature

    Women are less likely to be senior authors, invited to write in scientific journals, and to be cited in high impact journals. The aim of this study was to investigate trends in authorship and gender differences in peer-reviewed burn literature over 13 years. We performed a retrospective analysis of original research articles published from January 2009 to September 2021 in three burn journals. A gender determination application was used to categorize the gender of the first and senior author. Of the 3908 articles analyzed, 42.5% had a woman first author and 27.6% had a woman senior author. We identified 2029 unique senior authors, 29.0% of whom were women. Woman senior authorship was associated with increased odds of woman first authorship [OR = 2.31 (95% CI: 2.00, 2.67); P < .001]. The percentage of papers with a woman senior author increased from 17.8% in 2009 to 35.7% in 2021. If this 1.0% (95% CI: 0.50-1.51%) linear trend increase per year in woman senior authorship continues, we will expect to see equal proportions of woman and man senior authors in the included journals starting in 2037. The field of burn care is far from reaching gender parity with respect to authorship of peer-reviewed publications. Supporting and encouraging gender-concordant and discordant first:last authorship dyads in mentorship as well as redistributing obligations that may detract from authorship opportunities are potential ways to improve parity in authorship and academia.

    Gender disparities in authorship: A fifteen-year bibliometric analysis of interventional radiology journals Open PDF

    Introduction: This study explores the evolution of authorship dynamics in Interventional Radiology over a fifteen-year period, with a focus on gender disparities. It aims to illuminate the representation of female authors in high-impact IR journals, highlighting ongoing challenges in achieving gender diversity, especially in leadership roles. Methods: A bibliometric analysis was conducted on articles published from 2008 to 2023, systematically assessing trends in female first and last authorships, as well as their overall contribution to total authorship within prominent IR journals. Results: The analysis revealed a fluctuating trajectory in female authorship. Female first authorship peaked in 2011, followed by a decline through 2012-2013, and subsequently rebounded by 2023. Female last authorship exhibited a similar non-linear pattern, with an initial decline from 2008 to 2013, before gradually increasing in recent years. Overall, the presence of female authors in IR publications has steadily grown, with a notable rise in the proportion of publications featuring female authors and an increase in the average number of female contributors per article by 2023. Conclusion: Despite the overall growth in female authorship, significant disparities persist, particularly in senior authorship positions. These findings highlight the need for sustained efforts to address gender inequities in IR, emphasizing the importance of robust policies and support systems to promote women's advancement within the field. © 2024 Elsevier Inc.

    The contribution of formal and informal mentorship to faculty productivity: Views of faculty in public affairs programs Open PDF

    This study examines how mentorship opportunities contribute to the productivity and career growth of public affairs faculty, stratified by gender and race. The study uses primary data coming from an original survey administered at two different points in time (2017 and 2021) to faculty who are part of NASPAA member schools. Results indicate that women and faculty from racially under-represented groups are more likely to receive formal mentoring whereas men and white faculty are more likely to benefit from informal mentoring. Additionally, results show that the relationship between mentoring approaches and research effort differed by the faculty’s member gender and race with formal mentoring contributing to the research effort of men and white faculty across all academic ranks and university types, and informal mentoring contributing to the research effort of mid-career faculty of all genders and races. This study aims to inform individuals and universities about mentoring trends and contributions.

    Gender Prevalence and Trends in Otology and Neurotology Publications Open PDF

    Objective:To assess otology and neurotology authorship by gender, subject, and country of origin from 2000 to 2019.Study Design:Retrospective review of otology and neurotology publications in 2000, 2005, 2010, 2015, and 2019 from 10 prominent journals within otolaryngology.Main Outcome Measures:Demographics for first through third and final authors, including gender, degree, coauthorship, as well as number of authors, subject matter, and region of origin for each publication.Results:A total of 4,411 neurotology articles published in 2000, 2005, 2010, 2015, and 2019 were analyzed. During the study period, the proportion of female authors increased from 22.3% in 2000 to 33.9% in 2019 (p<0.0001). However, authorship position analysis demonstrated no increase in final female authorship (22.5% in 2019, from 19.4% in 2000, p=0.112). Geographic region analysis demonstrated a similar overall global trend toward an increase in female authors. When considering author gender by position, North America, the Middle East, and Africa failed to demonstrate significant increasing trends for female final authors. Female final authors were also significantly less likely to have medical degrees than final male authors, (37.4% versus 78.6%, respectfully, p=<0.0001). Finally, women published more often within the subjects of pediatrics and audiology (46.5% and 37.3% of final authors, p<0.0001).Conclusions:Female authorship in otology and neurotology has increased globally. With the predominant number of articles originating from North America, and articles originating from North America failing to demonstrate an increase in female final authorship, overall, final female authorship did not change during the study period.Professional Practice Gap and Educational Need:There is no comprehensive study exploring gender distribution within only the specialty of otology and neurotology. While it is known that more women are becoming otolaryngologists, it is unknown if this increase is reflected in otology and neurotology publications, domestically and internationally.Learning Objective:To understand if gender biases and/or differences exist within otology and neurotology publications.Desired Result:Identify trends in otology and neurotology publications to address particular barriers to female publication within the field. © 2021 Lippincott Williams and Wilkins.
