    Gender representation trends in cardiothoracic surgery journal editorial boards Open PDF

    Objective We aimed to characterize chronologic trends of gender composition of the editorial boards of major cardiothoracic surgery journals in the current era. Methods A cross-sectional analysis was performed of gender representation in editorial board members of 2 North American cardiothoracic surgery journals from 2008 to 2023. Member names and roles were collected from available monthly issues. Validated software programming was used to classify gender. The annual proportion of women representation was compared to the thoracic surgery workforce. Results During the study period, 558 individuals (3641 names) were identified, 14.3% of whom were women. The total number of editorial board women increased for both journals. The proportion of women also increased from 2.5% (3 out of 118) in 2008 to 17.8% (71 out of 399) in 2023 (P < .001), exceeding the percentage of women in the thoracic surgery workforce, which increased from 3.8% in 2007 to 8.3% in 2021 (P < .001). The average duration of participation was longer for men than for women (53.8 vs 44.5 months; P = .01). Women in editorial board senior roles also increased from 3.3% (1 out of 30) in 2008 to 28.6% (42 out of 147) in 2023 (P < .001), almost triple the increase in nondesignated roles from 2.3% (2 out of 88) in 2008 to 11.5% (29 out of 252) in 2023 (P < .001). Conclusions In recent years, the appointment of women to the editorial boards of high-impact cardiothoracic surgery journals and senior roles have proportionally exceeded the overall representation of women in cardiothoracic surgery. These findings indicate progress in inclusive efforts and offer insight toward reducing academic gender disparities.

    The Status of Women in Academic Medicine, and Particularly in Neurosurgery, in a Developing Country, Türkiye Open PDF

    OBJECTIVE: Women have always faced inequality in academic medical careers in Türkiye as well as the rest of the world. However, to date, no definitive data has been published regarding the representation rate of women in academic medicine in Türkiye. The aim of this study is to reveal the status of women in faculty and leadership positions in academic medicine in Türkiye. METHODS: This is a cross-sectional observational study conducted through websites of the Council of Higher Education (YOK) in Türkiye and medical faculties accredited by YOK as of October 2025. RESULTS: There were 132 medical schools in Türkiye. Women constituted 41% of all faculty members, 37% of professors, 42% of associate professors, 44% of assistant professors, 57% of lecturers, and 50% of residents in medical faculties in Türkiye. Among leadership positions, there were 26 (21%) women deans, and 98 (46%) women vice deans. Women comprised 35% of the faculty administrative committees and 35% of faculty boards. 25% of chairpersons of medical sciences divisions, and 35% of department heads were women. Regarding women in Neurosurgery, only 6% (n=25) of faculty and 5% (n=5) of department heads in neurosurgery were women. Among neurosurgery faculty, women comprised only 2% (n=3) of full professors, 6% (n=5) of associate professors, 9% (n=14) of assistant professors, and 27% (n=3) of lecturers. There were 45 (28%) female residents in neurosurgery. CONCLUSIONS: Gender equity has not yet been achieved in academic medicine in Türkiye. More effective policies promoting gender equality should be implemented by the authorities.

    Exploring gender disparities in academic orthopaedic surgery faculty: analyzing subspecialty and leadership diversity to foster inclusivity Open PDF

    Background The historic gap in gender diversity within orthopaedic surgery is widely acknowledged and continues to persist. The lack of female representation in orthopaedic surgery has been attributed to a variety of factors, including the absence of female mentors and leaders within the field. As such, we sought to examine the gender diversity among orthopaedic surgery faculty in various subspecialties at academic institutions and the distribution of female faculty in positions of leadership. Methods The American Medical Association Fellowship and Residency Electronic Interactive Database (FREIDA) was used to identify all allopathic orthopaedic surgery residency programs during the 2022 to 2023 academic year. The total number of faculty, and distribution of female faculty by subspecialty were collected from January to March 2023. The mean and percentage of female faculty in each subspecialty per program was calculated. Results The total number of orthopaedic surgery female faculty identified was 524. The subspecialty with the highest percentage of female faculty per program was pediatrics at 26.1% (148/511). Hand (18.6%; 113/511), oncology (19.2%; 38/511), foot and ankle (13.6%; 49/511), spine (3.9%; 21/511), shoulder and elbow (7.4%; 7/511) and adult reconstruction (3.7%; 24/511) had lower percentages of female faculty per program. A total of 52 (10.2%) female section chiefs were identified across all programs. Oncology had the highest percentage of female faculty represented in section leadership at 18.4% (7) and sports medicine had the lowest at 4.8% (4). Conclusion Gender diversity of faculty in orthopaedic surgery is low with adult reconstruction (3.7%), spine (3.9%), and shoulder and elbow (7.4%) having the lowest percentages of female faculty. The percentage of female faculty represented in section leadership is also lacking with a total of 52 (10.2%) female section chiefs identified across all programs. Increasing the number of females in leadership positions across all orthopaedic subspecialties may be one step in helping improve gender diversity in the field.

    Gender disparities in authorship in European cardio-thoracic journals Open PDF

    OBJECTIVES The goal was to evaluate gender authorship in 2 cardio-thoracic surgical journals. METHODS We performed a bibliometric analysis of all articles published from 2017 to 2022 in the European Journal of Cardio-Thoracic Surgery and the Interdisciplinary Cardiovascular and Thoracic Surgery. For each article, the gender and academic rank of the first, senior and corresponding authors were verified by Internet search, email contact or use of the application Genderize.io. Articles were categorized based on topic, type and country of origin. The Cochran-Armitage test was used to evaluate gender authorship trend over time. RESULTS A total of 5243 articles were included in the analysis. Women represented 18% of first authors, 7% of senior authors and 13% of corresponding authors; no trend was seen over time. Women represented 16% of first authors and 7% of senior authors in adult cardiac surgery, 23% of first authors and 9% of senior authors in congenital cardiac surgery and 19% of first authors and 8% of senior authors in thoracic surgery. Male first authors were more frequently full professors (17% vs 5%) and associate professors (16% vs 8%), and male senior authors were more frequently full professors (48% vs 31%) and associate professors (16% vs 8%) compared to female senior authors. CONCLUSIONS The proportion of female authors is significantly lower than that of male authors in highest-impact European cardio-thoracic surgery journals, and no significant increase in female authorship has been demonstrated in recent years. Increasing awareness of gender disparities is essential to facilitate equal career opportunities and academic advancement for women in cardio-thoracic surgery.

    Authorship Trends and Gender Concordance in Surgical Research

    INTRODUCTION: Building inclusive and diverse research teams is increasingly important in the era of team science. We explored authorship trends over the last 3 decades to assess progress in academic surgery. METHODS: Two general surgical journals (Journal of the American Medical Association Surgery and Annals of Surgery) and two nonsurgical multidisciplinary scientific journals (Nature and Science) were queried at 10-y intervals from 1991 to 2021. Data collected included total number of authors per article, instances of shared authorship, authors' gender, geographic location of the authors, and number of references per article. RESULTS: A total of 741 journal articles were examined. Over the last 3 decades, the number of women authors has nearly doubled in surgical journals and tripled in nonsurgical journals. The proportion of women authors per article, women first authors, and women senior authors all increased significantly, though women remain only 26% of all authors. Intriguingly, articles with a woman senior author had a significantly greater proportion of women coauthors than articles with a man senior author (55 ± 4% versus 12 ± 6%; P < 0.05) across all 3 decades in all journals. In 2021, only 10% of articles with a man senior author had an equal or greater proportion of women coauthors. CONCLUSIONS: Though there are now more women authors, we describe a pattern of gender concordance where women senior authors may publish with other women more than men senior authors. Our findings highlight both the progress made and potential for growth in achieving gender equity in academic surgery.

    Sticky Floor, Broken Ladder, and Glass Ceiling: Gender and Racial Trends Among Neurosurgery Residents

    Introduction Diversity and equity in academic medicine are critically important in improving healthcare standards and patient-related outcomes. Gender and racial disparities are some major challenges faced by the health system. This article reviews the gender and racial trends among residents of neurosurgery in the United States (US). Methods We retrospectively analyzed the data extracted from the Accreditation Council for Graduate Medical Education (ACGME)'s annual Data Resource Books from 2007 to 2019. ACGME cataloged gender as men and women and race/ethnicity was categorized as White/non-Hispanic, Asian or Pacific Island, Hispanic, Black/non-Hispanic, Native American/Alaskan, others, and unknown. Counts, proportions, relative, and absolute percentage changes were calculated to highlight trends in resident appointments over time and across the specialty of neurosurgery. Results The number of female residents increased steadily from 10.6% in 2007 to 19.3% in 2019; with an absolute increase of 8.74%, a relative increase of 63.9%, and a simultaneous decrease in male residents. When averaged across the nine-year study period, 51% of the study sample was White (non-Hispanic), followed by Asian/Pacific Islanders at 15.2%. The representation of Hispanics was 4.3%, Black/African Americans were 4.5%, Native Americans/Alaskans were 0.2%, and others were 8% of the total study population. Conclusion Our study concludes that gender and racial disparity persist within the neurosurgery residency training programs in the US. Concrete efforts at all academic levels are needed to provide greater support for the females and for the careers of underrepresented minority (URM) trainees to ensure their increased representation in neurosurgery.

    Trends in Female Plastic Surgery Resident Authorship - Signs of Changing Times? Open PDF

    OBJECTIVE: Gender discrepancies exist in academia for leadership positions, advancement opportunities, and research. As of 2019, the ratio of total male-to-female attending plastic and reconstructive surgeons was 4.8:1. However, the ratio of male-to-female residents in integrated plastic surgery programs fell to 1.3:1, indicating rising female representation. With more balanced gender distributions of residents, the authors sought to determine whether this translates to greater equality of opportunities and achievements. Specifically, this study compares the academic productivity of male and female integrated plastic surgery residents. METHODS: A list of integrated plastic surgery residency programs was obtained from the Accreditation Council for Graduate Medical Education website and ranked by reputation using the Doximity Residency Navigator. Integrated plastic surgery residents from 2019 to 2020 were identified via program websites and social media accounts. Works published during residency were identified through PubMed and Scopus from July 1 of each resident's intern year through August 10, 2020. Demographic variables for residents, including training class and medical school, as well as for programs, including geographic region, Doximity ranking, and medical school affiliation, were collected. Medical schools were ranked according to US News by research. Research productivity was assessed through the number of total research articles with authorship position (first, second, or last), the number of articles published in plastic surgery journals with the highest impact factors (Plastic and Reconstructive Surgery and Aesthetic Surgery Journal), and H-indices. Chi-Squared tests and Mann-Whitney U-tests were used to make comparisons between male and female residents (α = 0.05). RESULTS: In total, 931 residents in 81 integrated plastic surgery programs were identified, including 534 (57.4%) male and 397 (42.6%) female residents. There were no differences between male and female residents in terms of training year or program geography. Female residents were more likely to come from a top-50 medical school than males (54.7% vs. 48.1%, p = 0.049). There were no significant differences in gender distribution of residents from top-20 programs or programs affiliated with a top-20 medical school. The median (IQR) number of publications in total, and for each gender, was 3 (1-6). There was no difference in the number of total publications by training year by gender, besides the second-year resident class where male residents had a median (IQR) of 2 (1-4) compared to 1 (0-3) (p = 0.028). Male and female residents did not differ with regards to authorship position or proportion of times publishing in top journals. The distribution of H-indices for male residents was slightly higher than female residents (p = 0.003), but the median (IQR) was the same at 3 (1-5). CONCLUSIONS: Currently, male and female integrated plastic surgery residents have similar levels of academic productivity. This suggests that female representation is slowly increasing along the pipeline in academia, representing a paradigm shift from previous trends of gender inequality in plastic surgery. © 2021 Association of Program Directors in Surgery

    Female and low- and middle-income authorship trends in high-impact ENT journals (2011-2020) Open PDF

    INTRODUCTION: Despite a recent drive to increase diversity, the global academic workforce is skewed in favor of authors from high-income countries, and women are under-represented in the published medical literature. OBJECTIVES: To explore the trends in authorship of three high-impact otolaryngology journals over a ten-year period (2011-2020). METHODS: Journals selected: JAMA Otolaryngology-Head and Neck Surgery, The Laryngoscope and Rhinology. Articles were reviewed from four issues per journal per year, and data was collected on: time of publication; subspeciality; number of authors; sex of first and last authors; country of practice of first author and country where each study was conducted. Trends were examined though univariate and multivariate logistic regression models. RESULTS: 2998 articles were included. 93.9% of first authors and 94.2% of studies were from high-income countries.Women were first authors in 31.5% (n = 912) and senior authors in 18.4% (n = 524) of articles. Female first authorship significantly increased between 2011 and 2020 however female senior authorship remained the same. There have been no significant changes in the proportion of published articles from low-and middle-income countries (LMIC) over time (p = .65). Amongst the LMIC articles, 72% came from Brazil, Turkey or China and there were no published papers from countries with a low-income economy (gross national income per capita of $1085 or less). CONCLUSIONS: Although female first authorship has increased in the last decade, there has been minimal other demographic change in authorship over this time. High-impact otolaryngology journals poorly represent academia in low-and-middle income countries. There is a need for increased advocacy promoting gender and geographical research equity in academic medicine. LEVEL OF EVIDENCE: III.
